How It Works
No shipping is required for this purchase.
After your purchase, you will receive an email with setup instructions and a Health History Questionnaire. Your questionnaire must be completed before we can set up your app and personalize your experience. Once we receive your completed questionnaire, your account will be created and you'll receive instructions to access the app.
Refunds & Cancellations
Upfront App Plans (3, 6, or 12 Months)
Upfront plans are a one-time payment and do not automatically renew. Once your account has been created, all purchases are final and non-refundable.
Monthly Subscription Plans (3, 6, or 12 Month Commitments)
Once your account has been created, subscription payments are non-refundable. Your subscription will remain active for the full commitment period selected at checkout. After your commitment period is complete, your subscription will continue month-to-month until cancelled.
After your commitment period ends, you can cancel anytime through your subscription settings or by submitting a cancellation request at least 2 business days before your next billing date.
